⚠️ Important Submission Instructions – Request for Return of Weapons

To ensure your request is processed without delays, please follow these instructions carefully:

Download and complete the following required forms:
- Download Form 1: Consent for Mental Health Records Search – Please download and complete the required form here. Once completed, please upload in the provided submission box.
- Download Form 2: Certification for Return of Weapons – A link to download the form, and then: Please complete the provided form (Including questions #3, which is often overlooked) once completed, please upload in the provided submission box.
Upload the following required documents:
- Copy of your valid Driver’s License reflecting your current address (front and back)
- Copy of your valid Firearms ID Card reflecting your current address (front and back)
- Proof of ownership for any weapons you are requesting to be returned
- If weapons were seized for mental health reasons, upload any and all records related to your PESS evaluation, hospitalization, and treatment received since the seizure
- Any other documents you wish the Assistant Prosecutor to review
Ensure all forms are fully and accurately completed. Incomplete, unsigned, or missing information may delay the review process.
Do not submit your request without attaching all required forms and documents.
Requests submitted without attachments will not be processed.

This form will be electronically submitted to the Weapons Forfeiture Squad of the Ocean County Prosecutor’s Office.
