This form is required to request the return of a weapon(s). Please fill this form out completely and include all documents requested. Any information not included may result in a delay processing your request. This form will be electronically submitted to the Weapons Forfeiture Squad of the Ocean County Prosecutor’s Office to be processed. Your request must include either a valid phone number or email address.
